I hereby authorize OTC DAIHEN to perform field services on the described welding/robotic equipment. I acknowledge and agree that services are billed in accordance with the standard hourly rates, travel expenses, and part replacement costs, unless verified as an active warranty matter. No personnel will be dispatched without a validated Purchase Order (Note 1) on file.

Important Notices & Requirements:

  • Note 1 (PO Requirement): All service requests must be accompanied by a valid Purchase Order matching the estimated dispatch billing. Work cannot begin and technical specialists will not be scheduled prior to PO verification.
  • Note 2 (Warranty Information): Warranty service calls are subject to warranty validation terms. If a failure is found to be caused by external factors (improper setup, non-compatible wire, input line fluctuations), regular hourly billing will apply.
